2025 W-2 Forms
- 2025 W-2s were mailed by January 25th
- They were sent to the last known home address in UKG as of January 6, 2026
- This applies if you did not elect to receive your W-2 electronically
Need help or a prior year W-2?
For questions or to request a W-2 from a previous year, please email ESSPayroll@LSEnt.com and include the following:
- Your legal name
- Last four digits of your Social Security Number
- The center you worked at
- Your current mailing address
- The tax year you’re requesting
